To volunteer for FOCUS, go to http://www.focus-ga.org/ and print a volunteer application. Complete & fax to FOCUS at 770-234-9131. Someone from FOCUS will call or e-mail you with further information. We do require background checks on all volunteers over age 18.

Special Pops Tennis

Special Pops is starting up now and they have several locations around metro Atlanta, including Forsyth and Cherokee counties. Registration is free and equipment is also provided. Check it out. For more info and to sign up go to www.specialpopstennis.com.

Temporary Child Care Assistance Program

Temporary child care assistance program targets families affected by recession

The Georgia Department of Human Services (DHS) has created a new federally funded program called Temporary Child Care Assistance (TCCA) that will be operated under the state's Child Care Program. The program, which ends in September 2010, will provide time-limited assistance with child care costs for families that qualify.

To qualify for TCCA, a person must:

Be a Georgia resident

Be actively involved in either job readiness training, vocational training, technical school or college

Be unemployed or work part-time

Have income that falls within the income guidelines

Care for a child who is 12 years old or younger (or 17 years or younger if they have special needs)


To receive TCCA benefits, families must either apply directly online or call 1-866-371-8222 to receive a paper application by mail.

Autism/DD Family Ease Program

“AUTISM/DD FAMILY EASE” PROGRAM


This is an income qualifying service for children with Autism spectrum or developmental disability. If eligible, services include assistance with diapers, dental, additional OT, PT, speech, Transportation, Day Program, afterschool care, additional respite, medical supplies and equipment.



Dear Family,


Enclosed please find an application for Autism or DD Family Support/Ease Funding. Please look it over and fill it out as soon as you get a chance. There are several things that we require to process your application. You must have:

Completed Family Ease Application includes: Application, Financial Statement, **Proof of Income is REQUIRED**, Request Form, Family Ease Agreement, **PLEASE READ CAREFULLY**, and Authorization for release of Information. THE ENTIRE APPLICATION MUST BE FILLED OUT COMPLETELY!
Signature Required on Application
Documentation of Income (SSI statement, W-2, or copy of Pay Stub-Current Information IS REQUIRED)
Documentation of Developmental Disability (IEP, letter from Physician, Psychological evaluation)
Documentation from a Professional (Physician, Therapist, Doctor) stating the needs for goods/services (Letter of Recommendation)
Documentation showing that (canceled checks or receipts) you have paid for or are being billed for the requested items or services, along with Documentation from a Professional (Letter of Recommendation). If you have not yet purchased items or services, a letter from the provider or merchandiser stating costs including shipping and handling is required; along with a provider application from Georgia Community Support and Solutions is required for the provider you will be receiving items or services from.
For Autism, the family must live in Gwinnett, Rockdale, Newton, Dekalb, Fulton, Cobb or Douglas Counties.
For DD, the family must live in Gwinnett, Rockdale, Newton, Dekalb, Cobb or Douglas Counties.

It will be more important than ever to ensure that all required documents are returned with the application and that the request for support is comprehensive and precise. Please note, also, that each family is responsible for a portion based on a sliding scale.

Also included is a family information packet that explains the program further. This is for you to keep for your reference.

Kiddos Clubhouse Foundation Scholarship

2010 SCHOLARSHIP APPLICATION

Who may apply?
A family applying for this scholarship must:
• Currently live in the state of Georgia
• Demonstrate the need for funds due to lack of insurance, lack of insurance coverage, lack of other funding sources
• Be applying for a child under the age of 21
• Not have received a scholarship from Kiddos’ Clubhouse Foundation within the past two (2) years

What is the amount of the scholarship?
The scholarships to be awarded will provide up to $2,500 for payment of therapeutic services or equipment payable directly to the clinic where your child has received the services, licensed therapist or business or the equipment vending company that supplies the equipment.

What is the process?
• Download the application, please print it out, fill it out and sign it. Go to www.kiddosclubhousefoundation.org to download application.
• Applications will NOT be considered in the order in which they are received. All applications will be considered at the same time.
• All applications must be postmarked by March 31st, 2010

• Please allow 60 days past deadline date for processing, review and decision
• All applicants will be notified in writing of the status of their application within 60 days past the deadline date
• All scholarship monies must be utilized for purposes stated within one year from notification date.
• Families must be back in contact with a representative of Kiddos’ Clubhouse Foundation within 2 weeks of notification of scholarship (notifications will occur via phone call, email and/or regular mail).

Guidelines
• The monies may NOT be used to pay insurance premiums or medical bills.
• All invoices for therapy services provided or therapy equipment must be submitted to Kiddos’ Clubhouse Foundation. Funds may not be used for previous outstanding balances and must be for services past scholarship award date.
• Applications will only be accepted by mail. Applications that are sent via email or fax will not be considered.
• Applications that are incomplete or received without all required documentation will not be considered.


Kiddos' Clubhouse Foundation makes every effort to remain objective during the process for determining the recipient of the scholarships; therefore the first two pages of this document which contain all personal identification information will be removed and the application will receive a number code. Throughout the rest of the determination process the application will be referred to only by the number code. It is essential for purposes of objectivity and confidentiality that in filling out the remainder of this application names or other identifying information be left out. This refers to all sections, including the therapists and doctor sections and their names.
